What is a Community Paramedic Program?
Community Paramedicine (CP) is a community-based health care program that evolves based on each community's individual needs.
The Johnston County Community Paramedic Program is an innovative way to care for patients outside the hospital.
This Program is comprised of paramedics with 80 hours of specialized training in addition to their paramedic training. This expanded level of training allows the Community Paramedic to collaborate more effectively with all members of the patient's healthcare team in order to support the needs of each patient.
Similar programs in the United States have produced remarkable results.

Does this cost me money?
We are funded by the Duke Endowment Grant. Therefore, our program is at no cost to you.
Do you come to my home?
Yes! We make appointments at your convenience.
Do you come on weekends?
Yes! We are available seven days a week, from 7 am to 7 pm.
Do you come to my house in an ambulance?
No. We will come to your house in a Ford Explorer clearly marked as a Johnston County Community Paramedic.
Who qualifies for the program?
To qualify for the program, you must:
- Live in Johnston County.
- Be referred into the program by a case manager and have a diagnosis of Congestive Heart Failure, Chronic Obstructive Pulmonary Disease, or Diabetes.
- Have utilized EMS four or more times in a 30-day period.
- Have utilized the emergency department seven or more times in six months.
